Pre-participation medicals/screening – forms must be submitted as outlined below.
First-Year Student Athletes:
This includes undergrads, graduate students and transfer students. All first-year athletes require medical clearance from a physician prior to any training or practice.
- Complete the 2025-26 first year athlete medical form - First Year Athlete PPE Form
- You will need to download and have a physician complete the Medical Finding Form.
- Once the medical finding form is completed by a physician, please send a copy of the completed document to : fksmc.varsity@uwo.ca
- Once completed all forms are reviewed by the Fowler Kennedy Sports Medicine team. This process takes a few days thus the requirement to have the forms completed 1 week prior to your teams start date.

High risk teams – all athletes will need to be seen at your team specific scheduled intake day in addition to completing the required medical forms. High risk teams will get an update from their head coach directly with the team specific medical intake day and time.

Returning Student Athletes:
Applies to all returning varsity athletes.
- Please go to Pre-Participation Health Questionnaire 2025-26 and complete the online Returning Athlete PPE Form
- The form must be completed and submitted a minimum of 1 week prior to the start of your teams training camp/tryouts.

Students are covered under PurpleCARE from September 1st to August 31st
We are thrilled to announce that a new Benefits Management Platform will be launching on September 1st, 2025! This innovative system is designed to streamline your experience and provide enhanced features to manage your Health and Dental Plan more efficiently.
Important Changes: After August 31
st, 2025, students registered with a Canada Life account will no longer be able to log in using the existing Canada Life app or web portal. Additionally, the current Benefits Card will no longer be active for direct billing. Students will need to create a new account on the new Benefits Management Platform, which will retain the history of previous claims through Canada Life, and the new Benefit Cards.
Please note the following important dates:
- Pre-registration will open on August 18, 2025, and will be available exclusively to current eligible, active students.
- New students' registration is available September 1, 2025.

stof their incoming year. Potential athletes (Canadian and International) are encouraged to confirm their existing medical coverage prior to starting the tryout process should their sport specific training camp begin prior to September 1

***Please note if a student athlete fails to submit a completed form they are not to participate until their paperwork is completed and they are approved to train.
***Student Athletes that arrive at the first practice and do not have the forms and medical clearance from the physician completed will not be allowed to participate until the forms have been completed and they are approved to train.
